Karen Pryor Clicker Training Waiver of Liability Relating to Coronavirus/COVID-19 After 10/5/2022

Please complete the KPCT Covid-19 Liability Waiver to be eligible to attend your Dog Trainer Professional program in-person workshop. KPCT must have a completed waiver for each Dog Trainer Professional student, faculty and/or workshop guest before attending each workshop. Please reach out to [email protected] with any questions.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is highly contagious. The virus is believed to be spread through the air by infected persons to other people in close proximity, as well as by physical contact with contaminated surfaces, objects, or people. Infected people can be asymptomatic. COVID-19 can cause serious health complications, potentially life-threatening illness, and even death.

Karen Pryor Clicker Training Inc. and Sunshine Books, Inc. (together, “KPCT”) cannot prevent you from exposure to, contraction of, or spreading COVID-19 while attending workshops provided and sponsored by KPCT (“KPCT Workshop”). It is not possible to eliminate the presence of the disease.

Therefore, if you choose to attend a KPCT Workshop, you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself in order to attend the KPCT Dog Trainer Professional Workshop on the assigned dates.

I accept the risk of exposure to, infection by, and/or spread of COVID-19 in order to attend the KPCT Workshop.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against KPCT and its owners, officers, directors, managers, officials, agents, employees or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to attending the KPCT Workshop. I understand that this waiver means that I give up my right to bring claims including for personal injuries, death, disease, or property loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

CHOICE OF LAW: I understand and agree that the law of the Commonwealth of Massachusetts will apply to this Waiver.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE.

Student Protocols for Attendance at Workshop Sponsored by Karen Pryor Clicker Training/Sunshine Books, Inc.

As a condition of my in-person attendance at the KPCT Dog Trainer Professional Workshop(s), I attest that I have signed the Karen Pryor Clicker Training Waiver of Liability Relating to Coronavirus/COVID-19 and I agree to adhere to, and abide by, the protocols for a workshop(s) provided by KPCT, as described below:

If I am experiencing symptoms of COVID-19 I will take a Covid-19 CDC or FDA authorized (or local official if outside of the United States) self test (also known as “home tests” and “at home test” or “over the counter” tests). These symptoms include: fever, chills, cough, shortness of breath or difficulty breathing, fatigue, muscular or bodyache, headache, loss of taste or smell, sore throat, congestion, runny nose, nausea, vomiting, or diarrhea. I also agree to self-test if I know I have had a recent close exposure to a person who has tested positive.
- If I test positive; I will not attend the workshop five days from my positive test. On the sixth day and thereafter I may attend the workshop but wear a mask until I test negative.
- If I am experiencing symptoms but test negative, I agree to wear a mask until my symptoms dissipate. If asked, I agree to test while my symptoms persist.
I understand that certain locations or KPA faculty members may require masks regardless of #1 above. If a location or KPA faculty member requires masks, I will wear a mask covering my nose and mouth at all times while at the facility and attending the KPA workshop with the exception of briefly lowering or removing my face covering while actively eating or drinking but it must be worn between bites and sips. All face coverings must cover the nose and mouth and comply with CDC guidelines for acceptable face coverings.
I acknowledge that protocols may change or be added and agree to follow new guidelines or instructions that are provided.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THE STUDENT PROTOCOLS AND I AGREE TO ABIDE BY THEM DURING THE KPCT DOG TRAINER PROFESSIONAL WORKSHOP AND WHILE ON THE PREMISES.
